Individual Health Care Plans (IHPs) are designed for students who have medical conditions that require medical care at school. These plans are developed between the school nurse and parents. Although these plans do not require a physician's signature, medical documentation of the health condition and/or a Medication Prescriber/Parent Authorization (PPA) may be required. Once completed, these plans are shared with your child's teacher(s) and other school employees who have "the need to know" about your child's condition. Examples- bus drivers and cafeteria staff.
