Vaccine Reminder for Rising 6th Graders and ALL New Students

Dear Parents/Guardian of 6th Graders And Also All New Students

This is just a reminder to make that appointment for your 6th grader to get his/her Tdap vaccine before the start of the new school year.
The Tdap is mandatory of all 11 or 12 year olds entering the 6th grade in the fall of 2018. (This does not apply to anyone who will be 10 entering the 6th grade on August 7, 2018).
The Tdap vaccine may be obtained at his/her physician's office, a health department, or clinic.
Once your child has received his/her Tdap, please bring the updated Alabama Immunization Certificate to his/her new school. Students without the updated Tdap on file will not be given a schedule at the start of school.
We want to also remind parents/guardians of all new students to Madison City Schools, regardless of grade, to furnish a hard copy of their child's Alabama immunization certificate to his or her school. This is in addition to uploading the certificate during the registration process.
Thank you for your help in these matters.
