The Madison City School System is proud to participate in the National School Lunch Program. All City schools offer a complete breakfast and lunch meal each school day. Meals are planned by a Registered Dietitian and comply with standards set forth by the United States Department of Agriculture. The Madison City School Board encourages all students to support the school lunch program by purchasing a school-made meal each day. This site contains information designed to make this process simple and convenient.
The Madison City School System participates in the Offer vs. Serve program. All students in grades Pre-K through 12th grade shall be permitted to decline any one (1) of the four (4) items offered at breakfast; however, every student must select a fruit or fruit juice. Students may decline two (2) of the five (5) items offered at lunch; however, every student must select a fruit or a vegetable.

If your child was free or reduced in Madison City Schools during the 2015-2016 school year, they will be the same eligibility for the first 30 school days of the 2016-2017 school year. Every student should receive a paper copy of the application on their first day of school; however, it is only necessary to complete one application (online or paper) per household per year. If you feel that you need a paper application today, you may stop by the Madison City Schools Central Office at 211 Celtic Drive to pick up one or you may email the Child Nutrition bookkeeper at email@example.com and one will be emailed back to you.
The online application for Free and Reduced Price School Meals is available here.Applications for the 2017-2018 school year cannot be submitted before July 1, 2017. The 2017-2018 application will be posted online by July 5.
The Richard B. Russell National School Lunch Act requires the information on this application. You do not have to give the information, but if you do not, we cannot approve your child for free or reduced price meals. You must include the last four digits of the social security number of the adult household member who signs the application. The last four digits of the social security number is not required when you apply on behalf of a foster child or you list a Supplemental Nutrition Assistance Program (SNAP), Temporary Assistance for Needy Families (TANF) Program or Food Distribution Program on Indian Reservations (FDPIR) case number or other FDPIR identifier for your child or when you indicate that the adult household member signing the application does not have a social security number. We will use your information to determine if your child is eligible for free or reduced price meals, and for administration and enforcement of the lunch and breakfast programs. We MAY share your eligibility information with education, health, and nutrition programs to help them evaluate, fund, or determine benefits for their programs, auditors for program reviews, and law enforcement officials to help them look into violations of program rules. The U.S Department of Agriculture prohibits discrimination against its customers, employees, and applicants for employment on the bases of race, color, national origin, age, disability, sex, gender identity, religion, reprisal, and where applicable, political beliefs, marital status, familial or parental status, sexual orientation, or all or part of an individual’s income is derived from any public assistance program, or protected genetic information in employment or in any program or activity conducted or funded by the Department. (Not all prohibited bases will apply to all programs and/or employment activities.) You may also write a letter containing all of the information requested in the form.
